I've uploaded my latest version, taking into account Latios' points and including ossias in two places. I kept the first one consistent with the one in the image I posted, even if it's just for one note. Let me know if you think a more compact notation would be preferable. 
If it'd written out full floating ossia measures they'd have to be crammed into a very small space or a full second page would have to be added.

I have updated the sheet with LH notes in the bass clef and RH notes in the treble clef as well as the octave shifts.   
As for the hand-stretchy parts, I'm wondering if something like this would work.

I'll do that. I'm just too used to scores that doesn't prioritise performance instructions that much.

I trust your judgement and have removed the key signatures. 
I'm not sure whether (or how) they should be mixed with sharps or not, but I've stuck to flats.

The other two issues should be resolved as well.

Yeah, that wasn't intended at all. Not sure how it happened, but it's gone now.

The key signature should be G major and I think you can fit in the little figure/melody at the end of the second measure.

You're probably right; it's not there in the versions from later games in the series.   :P
I personally hear the G4 very clearly though, but it's probably the second harmonic of the bass, and spectrum analysis doesn't really clarify the situation.

I felt that the piece felt a bit empty when reduced to only two voices. 
Additionally I think that the # in the title should be changed to the more conventional No.

Don't know why I left it like that, but I've fixed it up now.
I also removed the time signature changes in the middle because I felt it wasn't really necessary.

I have considered notating it in Db Major (based on the final chord), C Major, or Gb Major, but I think the split double signatures makes the most sense and looks the cleanest.

Yeah, but "Fishy" is just what one person translated it as. The official title has nothing to do with fish, it just means "dubious" or "suspicious", so it feels very arbitrary.
Not that I wouldn't love to have it on the site :)
